When commencing at the Club parents are required to complete an Enrolment Form. This Form asks for details of any actions the Club may need to undertake (where possible) relating to medical, physical or intellectual conditions that may have a bearing on a child’s ability, safety or behaviour in a class situation.

We need to know what immediate actions are necessary so that our First Aid Officers can render appropriate ad to your child if necessary. If immediate medication is required we must have readily available on the premises. |
The medical information that we collect is only used so that we can provide immediate care to our gymnasts. The Office Manager is to maker a notation on the Class Roll to indicate that there is an existing condition and is to inform the coach when he/she takes the class. The coach is to be briefed by the Office Manager on the nature of the condition and is to be told what actions he/she is required to undertake if the child requires it.
If emergency medication, for example, use of an EpiPen in response anaphylactic shock brought on by a severe allergic reaction or Puffer in response to an asthma attack, is prescribed, then that medication must be given to the Office Manager by the parents or guardians, either at the start of each class that the child is present or the required dosage is to be left in the nominated container in the Office. The parents or guardians are responsible for ensuring the dosage is correct and that use-by date is always current.
If emergency first aid is required then details of the immediate actions are to be noted in a folder, with the gymnast’s name clearly marked, and it is to be placed in the nominated holder in the Office. The gymnast’s coach and all senior coaches will also be briefed on the requirements for immediate first aid and how it is to be administered so that they are fully aware in any situation that arises.
If emergency first aid is required, first aid will be administered in accordance with instructions given by the parents or guardians. An ambulance may be called and the gymnast will be transported directly to the Royal Canberra Hospital. The parents or guardians will be called and notified.
Our Enrolment Form also includes our Privacy Statement. The Club Committee will ensure that all Club staff abide by the statutes of the Privacy Act in managing any medical information that is collected.
